What to Bring to Your Appointment 

To make your visit as smooth as possible, please bring: 

  • A valid photo ID 
  • Your current vision or medical insurance card(s) 
  • A list of medications and supplements you take 
  • Your current glasses, contact lenses, or prescriptions 
  • Any paperwork from your referring provider, if applicable 

If you’re a new patient, please arrive 10–15 minutes early to complete intake forms.

We accept a wide range of vision and medical insurance plans for both routine eye exams and medical eye care. If you’re not sure whether your visit is covered, our team can help you verify eligibility and explain what to expect before your appointment.

For added convenience, we also accept CareCredit, giving patients flexible payment options to make quality eye care easier to manage.
